Move init-container feature from alpha to beta.

Moved init-container feature from alpha to beta.

In 1.3, an init container is specified with this annotation key
on the pod or pod template: `pods.alpha.kubernetes.io/init-containers`.

In 1.4, either that key or this key: pods.beta.kubernetes.io/init-containers`,
can be used.

When you GET an object, you will see both annotation keys with the same values.

You can safely roll back from 1.4 to 1.3, and things with init-containers
will still work (pods, deployments, etc).

If you are running 1.3, only use the alpha annotation, or it may be lost when
rolling forward.

The status has moved from annotation key
`pods.beta.kubernetes.io/init-container-statuses` to
`pods.beta.kubernetes.io/init-container-statuses`.
Any code that inspects this annotation should be changed to use the new key.

State of Initialization will continue to be reported in both pods.alpha.kubernetes.io/initialized
and in `podStatus.conditions.{status: "True", type: Initialized}`

Mini-design for this change:

Goals:
1. A user can create an object with the beta annotation
  on 1.4, and it works.  The fact that the annotation has beta
  in it communicates to the user that the feature is beta,
  and so the user should have confidence in using it.  Preferably,
  when the user gets the annotation back, he see the beta
  annotation.
1) If someone had an existing alpha object in their apiserver,
  such as a RS with a pod template with an init-containers
  annotation on it, it should continue to work (init containers
  run) when stack upgraded to 1.4.
2) If someone is using a chart or blog post that has alpha
  annotation on it and they create it on a 1.4 cluster, it should
  work.
3) If someone had something with an init container in 1.4
  and they roll back stack to 1.3, it should not silently stop
  working (init containers don't run anymore).

To meet all these, we mirror an absent beta label from the alpha
key and vice versa.  If they are out of sync, we use the alpha
one. We do this in conversion since there was already logic there.

In 1.3 code, all annotations are preserved across a round trip
(v1 -> api -> v1), and the alpha annotation turns into the internal
field that kubelet uses.

In 1.4 code, the alpha annotation is always preserved across
a round trip, and a beta annotation is always set equal to
the alpha one, after a round trip.

Currently, the kubelet always sees the object after a round trip
when it GETs it.  But, we don't want to rely on that behavior,
since it will break when fastpath is implemented.

So, we rely on this:
all objects either are created with an alpha annotation (1.3 or 1.4
code) or are created with a beta annotation under 1.4.  In the later
case, they are round tripped at creation time, and so get both
annotations.  So all subsequent GETs see both labels.

Add spec.nodeName and spec.serviceAccountName to downward env var
The serviceAccountName is occasionally useful for clients running on
Kube that need to know who they are when talking to other components.

The nodeName is useful for PetSet or DaemonSet pods that need to make
calls back to the API to fetch info about their node.

Both fields are immutable, and cannot easily be retrieved in another
way.

I made two commits: one with the actual change and the other with the auto-generated code. I think it's easier to review this way, but let me know if you prefer in some other way.

I haven't written any tests yet, I wanted to have a first glance and not write them till this (and the API) are more close to the "LGTM" :)

There are some things:
 * I'm not sure where to do the "AND 0777". I'll try to look better in the code base, but suggestions are always welcome :)
 * The write permission on group and others is not set when you do an `ls -l` on the running container. It does work with write permissions to the owner. Debugging seems to show that is something happening after this is correctly set on creation. Will look closer.
 * The default permission (when the new fields are not specified) are the same that on kubernetes v1.3
 * I do realize there are conflicts with master, but I think this is good enough to have a look. The conflicts is with the autog-enerated code, so the actual code is actually the same (and it takes like ~30 minutes to generate it here)
 * I didn't generate the docs (`generated-docs` and `generated-swagger-docs` from `hack/update-all.sh`) because my machine runs out of mem. So that's why it isn't in this first PR, will try to investigate and see why it happens.

Other than that, this works fine here with some silly scripts I did to create a secret&configmap&downwardAPI, a pod and check the file permissions. Tested the "defaultMode" and "mode" for all. But of course, will write tests once this is looking fine :)

568f4c2e63 Add mode permission bits to configmap, secrets and downwardAPI
This implements the proposal in:
docs/proposals/secret-configmap-downwarapi-file-mode.md

Fixes: #28317.

The mounttest image is updated so it returns the permissions of the linked file
and not the symlink itself.

Add support to quota pvc storage requests

Adds support to quota cumulative `PersistentVolumeClaim` storage requests in a namespace.

Per our chat today @markturansky @abhgupta - this is not done (lacks unit testing), but is functional.

This lets quota enforcement for `PersistentVolumeClaim` to occur at creation time.  Supporting bind time enforcement would require substantial more work.  It's possible this is sufficient for many, so I am opening it up for feedback.

In the future, I suspect we may want to treat local disk in a special manner, but that would have to be a different resource altogether (i.e. `requests.disk`) or something.

Example quota:

The first change reduces runtime of BenchmarkPodCopy-4 from `> 3500ns` to around `2300ns`.

The second change reduces runtime to around `1900ns`.

This drives conversion generation from file tags like:
  // +conversion-gen=k8s.io/my/internal/version
.. rather than hardcoded lists of packages.

The only net change in generated code can be explained as correct.  Previously
it didn't know that conversion was available.

dc10f10e48 Recreate the opt-in/opt-out logic for deepcopy
This is the last piece of Clayton's #26179 to be implemented with file tags.
All diffs are accounted for.  Followup will use this to streamline some
packages.

Also add some V(5) debugging - it was helpful in diagnosing various issues, it
may be helpful again.

28af54138d Use file tags to generate deep-copies
This drives most of the logic of deep-copy generation from tags like:
  // +deepcopy-gen=package
..rather than hardcoded lists of packages.  This will make it possible to
subsequently generate code ONLY for packages that need it *right now*, rather
than all of them always.

Also remove pkgs that really do not need deep-copies (no symbols used
anywhere).

d88fc84bae Make multi-key tags more explicit
This is in prep to simplify tag logic.  Don't rely on processing commas as new
tag delimiters.  Put new tags on new lines.  This had zero effect on generated
code (as intended).

57c3196914 Add support for manually defined DeepCopy methods
In bringing back Clayton's PR piece-by-piece this was almost as easy to
implement as his version, and is much more like what I think we should be
doing.

Specifically, any time which defines a .DeepCopy() method will have that method
called preferentially.  Otherwise we generate our own functions for
deep-copying.  This affected exactly one type - resource.Quantity.  In applying
this heuristic, several places in the generated code were simplified.

To achieve this I had to convert types.Type.Methods from a slice to a map,
which seems correct anyway (to do by-name lookups).

e18b2f3a2e Revert Clayton's #26179 so I can recreate it
His PR cam during the middle of this development cycle, and it was easier to
burn it down and recreate it than try to patch it into an existing series and
re-test every assumption.  This behavior will be re-introduced in subsequent
commits.

I was asked about the qos tier of a pod that specified 

`--requests=cpu=0,memory=0 --limits=cpu=100m,memory=1Gi`

and in just investigating current behavior, realized we should have an explicit test case to ensure that 0 values are preserved in defaulting passes, and that this is still a burstable pod (but the lowest for that tier as it related to eviction)

Make unversioned.ListMeta implement List. Update all the *List types so they implement GetListMeta.
This helps avoid using reflection to get list information.

Remove all unnecessary boilerplate, move the interfaces to the right
places, and add a test that verifies that objects implement one, the
other, but never both.

542f2dc708 Introduce new kubelet volume manager
This commit adds a new volume manager in kubelet that synchronizes
volume mount/unmount (and attach/detach, if attach/detach controller
is not enabled).

This eliminates the race conditions between the pod creation loop
and the orphaned volumes loops. It also removes the unmount/detach
from the `syncPod()` path so volume clean up never blocks the
`syncPod` loop.

9dbe943491 Attach/Detach Controller Kubelet Changes
This PR contains Kubelet changes to enable attach/detach controller control.
* It introduces a new "enable-controller-attach-detach" kubelet flag to
  enable control by controller. Default enabled.
* It removes all references "SafeToDetach" annoation from controller.
* It adds the new VolumesInUse field to the Node Status API object.
* It modifies the controller to use VolumesInUse instead of SafeToDetach
  annotation to gate detachment.
* There is a bug in node-problem-detector that causes VolumesInUse to
  get reset every 30 seconds. Issue https://github.com/kubernetes/node-problem-detector/issues/9
  opened to fix that.

- RouteController will set it to "false" when the route is created
- Scheduler is scheduling only on nodes that doesn't have "NetworkNotReady ==true" condition

Refactor persistent volume controller

Here is complete persistent controller as designed in https://github.com/pmorie/pv-haxxz/blob/master/controller.go

It's feature complete and compatible with current binder/recycler/provisioner. No new features, it *should* be much more stable and predictable.

Testing
--
The unit test framework is quite complicated, still it was necessary to reach reasonable coverage (78% in `persistentvolume_controller.go`). The untested part are error cases, which are quite hard to test in reasonable way - sure, I can inject a VersionConflictError on any object update and check the error bubbles up to appropriate places, but the real test would be to run `syncClaim`/`syncVolume` again and check it recovers appropriately from the error in the next periodic sync. That's the hard part.

Organization
---
The PR starts with `rm -rf kubernetes/pkg/controller/persistentvolume`. I find it easier to read when I see only the new controller without old pieces scattered around.
[`types.go` from the old controller is reused to speed up matching a bit, the code looks solid and has 95% unit test coverage].

I tried to split the PR into smaller patches, let me know what you think.

ebe56f5ff9 Extend the current secrets mounting to volume implementation with key to path mapping.
The key to path mapping allows pod to specify different name (thus location) of each secret.
At the same time refactor the volume plugin to use AtomicWritter to project secrets to files in a volume.

Update e2e Secrets test, the secret file permission has changed from 0444 to 0644
Remove TestPluginIdempotent as the AtomicWritter is responsible for secret creation

Migrate to the new conversion generator - part1

This PR contains two commits:
- few more fixes to the generator
- migration of the pkg/api/v1 to use the new generator

The second commit is big, but I reviewed the changes and they contain:
- conversions between types that we didn't even generating conversion between
- changes in how we handle maps/pointers/slices - previously we were explicitly referencing fields, now we are using "shadowing in, out" to make the code more generic
- lack of auto-generated method for ReplicationControllerSpec (because these types are different (*int vs int for Replicas) and a preexisting conversion already exists

Most of issues in the first commit (e.g. adding references to "in" and "out" for slices/maps/points) were discovered by our tests. So I'm pretty confident that this change is correct now.

40c97ddd5f Remove validation of *VolumeSource.FSType.
Most volume plugins use SafeFormatAndMount, which uses ext4 by default.

FlexVolume plugin has FSType attribute 'omitempty', so reflect it in the
description of the type.

c1d932e44a Switch API objects to not register per version codecs
Remove Codec from versionInterfaces in meta (RESTMapper is now agnostic
to codec and serialization). Register api/latest.Codecs as the codec
factory and use latest.Codecs.LegacyCodec(version) as an equvialent to

Enforce minimum resource granularity of milli-{core, bytes} for Storage,

6e46fa1fd4 Add support for open-iscsi transports.
This enables use of software or hardware transports viz. be2iscsi,
bnx2i, cxgb3i, cxgb4i, qla4xx, iser and ocs. The default transport
(tcp) happens to be called "default".

Use of non-default transports changes the disk path to the following format:
/dev/disk/by-path/pci-<pci_id>-ip-<portal>-iscsi-<iqn>-lun-<lun_id>

e6df0b1a24 Convert validation to use FieldPath
Before this change we have a mish-mash of ways to pass field names around for
error generation.  Sometimes string fieldnames, sometimes .Prefix(), sometimes
neither, often wrong names or not indexed when it should be.

Instead of that mess, this is part one of a couple of commits that will make it
more strongly typed and hopefully encourage correct behavior.  At least you
will have to think about field names, which is better than nothing.

It turned out to be really hard to do this incrementally.

1d592e4c28 Unversioned types should not use ambiguous go-int
All external types that are not int64 are now marked as int32,
including
IntOrString. Prober is now int32 (43 years should be enough of an initial
probe time for anyone).

1e88a682da Add liveness/readiness probe parameters
- PeriodSeconds - How often to probe
- SuccessThreshold - Number of successful probes to go from failure to success state
- FailureThreshold - Number of failing probes to go from success to failure state

This commit includes to changes in behavior:

1. InitialDelaySeconds now defaults to 10 seconds, rather than the
kubelet sync interval (although that also defaults to 10 seconds).
2. Prober only retries on probe error, not failure. To compensate, the
default FailureThreshold is set to the maxRetries, 3.

The functions would default values added after v1.0, so that kubelet wouldn't
fail at comparing the mirror pods against their associated static pods.

This code would need to be maintained if we ever change the default value in
Pod until we drop support for v1.0 nodes.

fa39c2b032 Add flocker volume plugin
Flocker [1] is an open-source container data volume manager for
Dockerized applications.

This PR adds a volume plugin for Flocker.
The plugin interfaces the Flocker Control Service REST API [2] to
attachment attach the volume to the pod.

Each kubelet host should run Flocker agents (Container Agent and Dataset
Agent).

The kubelet will also require environment variables that contain the
host and port of the Flocker Control Service. (see Flocker architecture
[3] for more).

- `FLOCKER_CONTROL_SERVICE_HOST`
- `FLOCKER_CONTROL_SERVICE_PORT`

The contribution introduces a new 'flocker' volume type to the API with
fields:

- `datasetName`: which indicates the name of the dataset in Flocker
  added to metadata;
- `size`: a human-readable number that indicates the maximum size of the
  requested dataset.

fd91d3f0ec Make patch-up conversions simpler
Rather than an "all or nothing" approach to defining a custom conversion
function (which seems destined to cause problems eventually), this is an
attempt to make it possible to call the auto-generated code and then "fix it
up".

Specifically, consider you have a fooBar struct.  If you don't define a
conversion for FooBar, you will get a generated function like:
    convert_v1_FooBar_To_api_FooBar()

Before this PR, if you define your own conversion function, you get no
generated function.  After this PR you get:
    autoconvert_v1_FooBar_To_api_FooBar()
...which you can call yourself in your custom function.

f21d9ac9e4 Support pods with containers using host ipc
Add a HostIPC field to the Pod Spec to create containers sharing
the same ipc of the host.

This feature must be explicitly enabled in apiserver using the

bf2decce81 Add NotReadyAddresses to Endpoints
In many cases clients may wish to view not ready addresses for endpoints
in order to do set membership prior to a pod being ready. For instance,
a pod that uses the service endpoints to connect to other pods under
the same service, but does not want to signal ready before it has
contacted at least a minimal number of other pods.

This is backwards compatible with old servers and clients. There is
an additional cost in size of endpoints before services ramp up, which
will add minor CPU and memory use for services that have a significant
number of pods which have not become ready.

Getting the public IP a container is supposed to use is O(hard),
and usually involves ugly gyrations in python or with interfaces.
Using the downward API means that the IP Kube is announcing to
other endpoints is also visible inside the container for pods to
identify themselves.

bc8bc37282 Properly handle nil interfaces in DeepCopy.
Running reflect.ValueOf(X) where X is a nil interface will return
a zero Value. We cannot get the type (because no concrete type is
known) and cannot check if the Value is nil later on due to the way
reflect.Value works. So we should handle this case by immediately
returning nil. We cannot type-assert a nil interface to another
interface type (as no concrete type is assigned), so we must add
another check to see if the returned interface is nil.

72ee028cab Gracefully delete pods from the Kubelet
This commit wires together the graceful delete option for pods
on the Kubelet.  When a pod is deleted on the API server, a
grace period is calculated that is based on the
Pod.Spec.TerminationGracePeriodInSeconds, the user's provided grace
period, or a default.  The grace period can only shrink once set.
The value provided by the user (or the default) is set onto metadata
as DeletionGracePeriod.

When the Kubelet sees a pod with DeletionTimestamp set, it uses the
value of ObjectMeta.GracePeriodSeconds as the grace period
sent to Docker.  When updating status, if the pod has DeletionTimestamp
set and all containers are terminated, the Kubelet will update the
status one last time and then invoke Delete(pod, grace: 0) to
clean up the pod immediately.

Added the api/v1/v1.json file to the hack/update-swagger-spec.sh script so it gets updated and built.

Fixes #5057

25ada0e118 Inconsistency in api terminology of pod conditions
While nodes returns the "conditions" key in api requests, pods returns
"Condition". Also, from pods states docs it seems that the right
terminology should be "conditions".

Fixes #8038

* Add an allocator which saves state in etcd
* Perform PortalIP allocation check on startup and periodically afterwards

Also expose methods in master for downstream components to handle IP allocation
/ master registration themselves.
