Uses k8s.io/client-go/util/flowcontrol; implementation inspired by GCE
cloudprovider backoff.
**What this PR does / why we need it**:
The existing azure cloudprovider implementation has no guard rails in place to adapt to unexpected underlying operational conditions (i.e., clogs in resource plumbing between k8s runtime and the cloud API). The purpose of these changes is to support exponential backoff wrapping around API calls; and to support targeted rate limiting. Both of these options are configurable via `--cloud-config`.
Implementation inspired by the GCE's use of `k8s.io/client-go/util/flowcontrol` and `k8s.io/apimachinery/pkg/util/wait`, this PR likewise uses `flowcontrol` for rate limiting; and `wait` to thinly wrap backoff retry attempts to the API.

AWS: consider instances of all states in DisksAreAttached, not just "running"
Require callers of `getInstancesByNodeNames(Cached)` to specify the states they want to filter instances by, if any. DisksAreAttached, cannot only get "running" instances because of the following attach/detach bug we discovered:
1. Node A stops (or reboots) and stays down for x amount of time
2. Kube reschedules all pods to different nodes; the ones using ebs volumes cannot run because their volumes are still attached to node A
3. Verify volumes are attached check happens while node A is down
4. Since aws ebs bulk verify filters by running nodes, it assumes the volumes attached to node A are detached and removes them all from ASW
5. Node A comes back; its volumes are still attached to it but the attach detach controller has removed them all from asw and so will never detach them even though they are no longer desired on this node and in fact desired elsewhere
6. Pods cannot run because their volumes are still attached to node A
So the idea here is to remove the wrong assumption that callers of `getInstancesByNodeNames(Cached)` only want "running" nodes.
I hope this isn't too confusing, open to alternative ways of fixing the bug + making the code nice.

This is another attempt at https://github.com/kubernetes/kubernetes/pull/38702 . We have observed that `gce.service.Disks.Insert(gce.projectID, zone, diskToCreate).Do()` instantly gets an error response of alreadyExists, so we must check for it.
I am not sure if we still need to check for the error after `waitForZoneOp`; I think that if there is an alreadyExists error, the `Do()` above will always respond with it instantly. But because I'm not sure, and to be safe, I will leave it.

**What this PR does / why we need it**:
Improves performance for Cinder volume attach/detach calls.
Currently when Cinder volumes are attached or detached, functions try to retrieve details about the volume from the Nova API. Because some only have the volume name not its UUID, they use the list function in gophercloud to iterate over all volumes to find a match. This incurs severe performance problems on OpenStack projects with lots of volumes (sometimes thousands) since it needs to send a new request when the current page does not contain a match. A better way of doing this is use the `?name=XXX` query parameter to refine the results.

There were 2 ways of addressing this problem:
1. Use the `name` query parameter
2. Instead of using the list function, switch to using volume UUIDs and use the GET function instead. You'd need to change the signature of a few functions though, such as [`DeleteVolume`](https://github.com/kubernetes/kubernetes/blob/master/pkg/volume/cinder/cinder.go#L49), so I'm not sure how backwards compatible that is.
Since #1 does effectively the same as #2, I went with it because it ensures BC.
One assumption that is made is that the `volumeName` being retrieved matches exactly the name of the volume in Cinder. I'm not sure how accurate that is, but I see no reason why cloud providers would want to append/prefix things arbitrarily.

vSphere storage policy support for dynamic volume provisioning
Till now, vSphere cloud provider provides support to configure persistent volume with VSAN storage capabilities - kubernetes#42974. Right now this only works with VSAN.
Also there might be other use cases:
- The user might need a way to configure a policy on other datastores like VMFS, NFS etc.
- Use Storage IO control, VMCrypt policies for a persistent disk.
We can achieve about 2 use cases by using existing storage policies which are already created on vCenter using the Storage Policy Based Management service. The user will specify the SPBM policy ID as part of dynamic provisioning
- resultant persistent volume will have the policy configured with it.
- The persistent volume will be created on the compatible datastore that satisfies the storage policy requirements.
- If there are multiple compatible datastores, the datastore with the max free space would be chosen by default.
- If the user specifies the datastore along with the storage policy ID, the volume will created on this datastore if its compatible. In case if the user specified datastore is incompatible, it would error out the reasons for incompatibility to the user.
- Also, the user will be able to see the associations of persistent volume object with the policy on the vCenter once the volume is attached to the node.
For instance in the below example, the volume will created on a compatible datastore with max free space that satisfies the "Gold" storage policy requirements.

Fixing VolumesAreAttached and DisksAreAttached functions in vSphere
**What this PR does / why we need it**:
In the vSphere HA, when node fail over happens, node VM momentarily goes in to “not connected” state. During this time, if kubernetes calls VolumesAreAttached function, we are returning incorrect map, with status for volume set to false - detached state.
Volumes attached to previous nodes, requires to be detached before they can attach to the new node. Kubernetes attempt to check volume attachment. When node VM is not accessible or for any reason we cannot determine disk is attached, we were returning a Map of volumepath and its attachment status set to false. This was misinterpreted as disks are already detached from the node and Kubernetes was marking volumes as detached after orphaned pod is cleaned up. This causes volumes to remain attached to previous node, and pod creation always remains in the “containercreating” state. Since both the node are powered on, volumes can not be attached to new node.

Filter out IPV6 addresses from NodeAddresses() returned by vSphere
The vSphere CP returns both IPV6 and IPV4 addresses for a Node as part of NodeAddresses() implementation. However, Kubelet fails due to duplicate api.NodeAddress value when the node has an IPV6 address associated with it. This issue is tracked in #42690. The following are observed:
- when we enabled the logs and checked the addresses sent by vSphere CP to Kubelet, we don't see any duplicate addresses at all.
- Also, kubelet_node_status doesn’t receive any duplicate address from cloud provider.
However, when we filter out the IPV6 addresses and only return IPV4 addresses to the Kubelet, it works perfectly fine.
Even though the Kubelet receives the non-duplicate node-addresses, it still errors out with duplicate node addresses. It might be an issue when kubelet propagates these addresses to API server (or) API server is enable to handle IPV6 addresses.

In this new solution, if we leave it empty the algorithm will choose the zone for the cinder drive similar style like in aws and gce storageclass solutions.

If this example is deployed it will automatically create one replica per AZ. This helps us a lot making HA databases.
Current storageclass for cinder is not perfect in case of statefulsets. Lets assume that cinder storageclass is defined to be in zone called nova, but because labels are not added to pv - pods can be started in any zone. The problem is that at least in our openstack it is not possible to use cinder drive located in zone x from zone y. However, should we have possibility to choose between cross-zone cinder mounts or not? Imo it is not good way of doing things that they mount volume from another zone where the pod is located(means more network traffic between zones)? What you think? Current new solution does not allow that anymore (should we have possibility to allow it? it means removing the labels from pv).
There might be some things that needs to be fixed still in this release and I need help for that. Some parts of the code is not perfect.
Issues what i am thinking about (I need some help for these):
1) Can everybody see in openstack what AZ their servers are? Can there be like access policy that do not show that? If AZ is not found from server specs, I have no idea how the code behaves.
2) In GetAllZones() function, is it really needed to make new serviceclient using openstack.NewComputeV2 or could I somehow use existing one
3) This fetches all servers from some openstack tenant(project). However, in some cases kubernetes is maybe deployed only to specific zone. If kube servers are located for instance in zone 1, and then there are another servers in same tenant in zone 2. There might be usecase that cinder drive is provisioned to zone-2 but it cannot start pod, because kubernetes does not have any nodes in zone-2. Could we have better way to fetch kubernetes nodes zones? Currently that information is not added to kubernetes node labels automatically in openstack (which should I think). I have added those labels manually to nodes. If that zone information is not added to nodes, the new solution does not start stateful pods at all, because it cannot target pods.

In AWS environment when attach fails on the node
lets not use device from the pool. This makes sure
that a bigger pool of devices is available.
I changed the function signature to contain protocol, port, and path.
When the service has a health check path and port set it will create an
HTTP health check that corresponds to the port and path. If those are
not set it will create a standard TCP health check on the first port
from the listeners that is not nil. As far as I know, there is no way to
tell if a Health Check should be HTTP vs HTTPS.

This PR is to add support for Azure internal load balancer
Currently when exposing a serivce with LoadBalancer type, Azure provider would assume that it requires a public load balancer.
Thus it will request a public IP address resource, and expose the service via that public IP.
In this case we're not able to apply private IP addresses (within the cluster virtual network) for the service.

2. For a single Azure LoadBalancer, all frontend ip should reference either a subnet or publicIpAddress, which means that it could be either an Internet facing load balancer or an internal one.
For current provider, it would create an Azure LoadBalancer with generated '${loadBalancerName}' for all services with 'LoadBalancer' type.
This PR introduces name '${loadBalancerName}-internal' for a separate Azure Load Balancer resource, used by all the service that requires internal load balancers.
3. This PR introduces a new annotation for the internal load balancer type behaviour:
a. When the annotaion value is set to 'false' or not set, it falls back to the original behaviour, assuming that user is requesting a public load balancer;
b. When the annotaion value is set to 'true', the following rule applies depending on 'loadBalancerIP' field on ServiceSpec:
- If 'loadBalancerIP' is not set, it will create a load balancer rule with dynamic assigned frontend IP under the cluster subnet;
- If 'loadBalancerIP' is set, it will create a load balancer rule with the frontend IP set to the given value. If the given value is not valid, that is, it does not falls into the cluster subnet range, then the creation will fail.
4. Users may change the load balancer type by applying the annotation to the service at runtime.
In this case, the load balancer rule would need to be 'switched' between the internal one and external one.
For example, it we have a service with internal load balancer, and then user removes the annotation, making it to a public one. Before we creating rules in the public Azure LoadBalancer, we'll need to clean up rules in the internal Azure LoadBalancer.

Remove the dependency of login information on worker nodes for vsphere cloud provider:
1. VM Name is required to be set in the cloud provider configuration file.
2. Remove the requirement of login for Instance functions when querying local node information.
The cloudprovider is being refactored out of kubernetes core. This is being
done by moving all the cloud-specific calls from kube-apiserver, kubelet and
kube-controller-manager into a separately maintained binary(by vendors) called
cloud-controller-manager. The Kubelet relies on the cloudprovider to detect information
about the node that it is running on. Some of the cloudproviders worked by
querying local information to obtain this information. In the new world of things,
local information cannot be relied on, since cloud-controller-manager will not
run on every node. Only one active instance of it will be run in the cluster.
Today, all calls to the cloudprovider are based on the nodename. Nodenames are
unqiue within the kubernetes cluster, but generally not unique within the cloud.
This model of addressing nodes by nodename will not work in the future because
local services cannot be queried to uniquely identify a node in the cloud. Therefore,
I propose that we perform all cloudprovider calls based on ProviderID. This ID is
a unique identifier for identifying a node on an external database (such as
the instanceID in aws cloud).

VSAN policy support for storage volume provisioning inside kubernetes
The vsphere users will have the ability to specify custom Virtual SAN Storage Capabilities during dynamic volume provisioning. You can now define storage requirements, such as performance and availability, in the form of storage capabilities during dynamic volume provisioning. The storage capability requirements are converted into a Virtual SAN policy which are then pushed down to the Virtual SAN layer when a storage volume (virtual disk) is being created. The virtual disk is distributed across the Virtual SAN datastore to meet the requirements.
For example, User creates a storage class with VSAN storage capabilities:

Current vSphere Cloud provider doesn't allow a user to select a datastore for dynamic provisioning. All the volumes are created in default datastore provided by the user in the global vsphere configuration file.
With this fix, the user will be able to provide the datastore in the storage class definition. This will allow the volumes to be created in the datastore specified by the user in the storage class definition. This field is optional. If no datastore is specified, the volume will be created in the default datastore specified in the global config file.
For example:

GCE PD names generated by Kubernetes are guaranteed to be unique - they
contain name of the cluster and UID of the PVC that is behind it.
Presence of a GCE PD that has the same name as we want to provision
indicates that previous provisioning did not go well and most probably
the controller manager process was restarted in the meantime.
Kubernetes should reuse this volume and not provision a new one.

2. Create PVC which uses the StorageClass created in step 1.
3. Wait for PV to be provisioned.
4. Wait for PVC's status to become Bound

6. Wait for Disk to be attached to the node.
7. Get node VM's devices and find PV's Volume Disk.
8. Get Backing Info of the Volume Disk and obtain Property of `VirtualDiskFlatVer2BackingInfo` - `EagerlyScrub` and `ThinProvisioned`
9. Based on the value of `EagerlyScrub` and `ThinProvisioned`, verify if diskformat is correct.
10. Delete POD and Wait for Volume Disk to be detached from the Node.

Retrieves the VM's UUID in one of two methods - either by a `vm-uuid` entry in the cloud config file on the VM, or via sysfs. The sysfs route requires root access, but restores the previous functionality.
Multiple VMs in a vCenter cluster can share an IP address - for example, if you have multiple VM networks, but they're all isolated and use the same address range. Additionally, flannel network address ranges can overlap.
vSphere seems to have a limitation of reporting no more than 16 interfaces from a virtual machine, so it's possible that the IP address list on a VM is completely untrustworthy anyhow - it can either be empty (because the 16 interfaces it found were veth interfaces with no IP address), or it can report the flannel IP.

Fix for detach volume when node is not present/ powered off
Fixes#33061
When a vm is reported as no longer present in cloud provider and is deleted by node controller, there are no attempts to detach respective volumes. For example, if a VM is powered off or paused, and pods are migrated to other nodes. In the case of vSphere, the VM cannot be started again because the VM still holds mount points to volumes that are now mounted to other VMs.
In order to re-join this node again, you will have to manually detach these volumes from the powered off vm before starting it.
The current fix will make sure the mount points are deleted when the VM is powered off. Since all the mount points are deleted, the VM can be powered on again.
This is a workaround proposal only. I still don't see the kubernetes issuing a detach request to the vsphere cloud provider which should be the case. (Details in original issue #33061 )

In an effort to expand the existing pool of reviewers and establish a
two-tiered review process (first someone lgtms and then someone
experienced in the project approves), we are adding new reviewers to
existing owners files.
If You Care About the Process:

We did this by algorithmically figuring out who’s contributed code to
the project and in what directories. Unfortunately, that doesn’t work
well: people that have made mechanical code changes (e.g change the
copyright header across all directories) end up as reviewers in lots of
places.
Instead of using pure commit data, we generated an excessively large
list of reviewers and pruned based on all time commit data, recent
commit data and review data (number of PRs commented on).
At this point we have a decent list of reviewers, but it needs one last
pass for fine tuning.

As an owner of a sig/directory and a leader of the project, here’s what
we need from you:
1. Use PR https://github.com/kubernetes/kubernetes/pull/35715 as an example.
2. The pull-request is made editable, please edit the `OWNERS` file to
remove the names of people that shouldn't be reviewing code in the
future in the **reviewers** section. You probably do NOT need to modify
the **approvers** section. Names asre sorted by relevance, using some
secret statistics.
3. Notify me if you want some OWNERS file to be removed. Being an
approver or reviewer of a parent directory makes you a reviewer/approver
of the subdirectories too, so not all OWNERS files may be necessary.
4. Please use ALIAS if you want to use the same list of people over and
over again (don't hesitate to ask me for help, or use the pull-request
above as an example)

AWS: Remove duplicate calls to DescribeInstance during volume operations
This change removes all duplicate calls to describeInstance
from aws volume code path.
**What this PR does / why we need it**:
This PR removes the duplicate calls present in disk check code paths in AWS. I can confirm that `getAWSInstance` actually returns all instance information already and hence there is no need of making separate `describeInstance` call.

Means we can run in newly announced regions without a code change.
We don't register the ECR provider in new regions, so we will still need
a code change for now.
This also means we do trust config / instance metadata, and don't reject
incorrectly configured zones.
Fix#35014
The default polling delay of 1 minute results in very long delays when
an Azure Disk is attached to a node. It gets worse as go-autorest
doubles the default delay to 2 minutes.

Only the PollingDelay for VirtualMachinesClient is modified here to
avoid too much pressure on Azure quotas.

AWS: Add exponential backoff to waitForAttachmentStatus() and createTags()
We should use exponential backoff while waiting for a volume to get attached/detached to/from a node. This will lower AWS load and reduce API call throttling.

GCE PD names generated by Kubernetes are guaranteed to be unique - they
contain name of the cluster and UID of the PVC that is behind it.
Presence of a GCE PD that has the same name as we want to provision
indicates that previous provisioning did not go well and most probably
the controller manager process was restarted in the meantime.
Kubernetes should reuse this volume and not provision a new one.

This patch adds a device name allocator that tries to find a name that's next to the last used device name instead of using the first available one. This way we will loop through all device names ("xvdba" .. "xvdzz") before a device name is reused.

Remove Azure Subnet RouteTable check
**What this PR does / why we need it**:
PR Removes the subnet configuration check for Azure cloudprovider. The subnet check ensures that the subnet is associated with the Route Table. However if the VNET is in a different Azure Resource Group then the check fails, even if the subnet is already valid. This a stop gap fix, to allow Kubernetes to be deployed to Custom VNETs in Azure, that may reside in a different resource group to the cluster.
