zipl: rename PROGRAM_TABLE_* to {LEGACY}_BLKPTR_FORMAT_ID

Browse Source 
In preparation of decoupling the program table position from
the block pointer format, rename:

PROGRAM_TABLE_0 to LEGACY_BLKPTR_FORMAT_ID
PRAGRAM_TABLE_1 to BLKPTR_FORMAT_ID
